About the Role
• We’re excited to welcome new Great Mates to our growing team. We currently have multiple Clinical Lifestyle Assistant (Community Worker) opportunities available across the Gold Coast.
• In this rewarding and diverse role, you will support participants in our Supported Independent Living (SIL) environments through day-to-day care, life skill development, and connection to community networks. You’ll contribute to enhancing wellbeing, independence and inclusion while also assisting in community access, advocacy, and service coordination.
• This position works across a 7-day roster and suits those who thrive on variety, love working with people, and bring initiative and empathy to everything they do.
Key Responsibilities
• Supporting participants with diverse needs (including complex behaviours and health conditions)
• Implementing individualised support, behaviour, and healthcare plans
• Conducting interviews to evaluate the nature and scope of clients’ challenges
• Facilitating participation in community life, including access to education, employment, and local activities
• Connecting clients with relevant external agencies for further assistance and support
• Developing programs that build life skills (e.g. budgeting, cooking, public transport, social engagement)
• Advocating for participant needs and promoting community inclusion and social connections
• Working with families, community groups and external services to improve outcomes
• Tracking client progress and preparing regular progress reports
• Maintaining accurate documentation including case notes, incident reports, and service feedback
• Contributing to a safe, inclusive and culturally responsive environment
• Identifying clients’ needs and designing, developing, and delivering tailored educational, training, and support programs
About You
• Experience working with people with disability, mental health conditions, and/or complex needs
• Confident supporting participants in both home and community settings
• Understanding of person-centred practice and community development principles
• Excellent communication and problem-solving skills
• A can-do attitude with a commitment to ongoing learning
• Ability to work flexible hours including active overnight shifts
• Diploma in Individual Support or equivalent qualification in community services
• Current First Aid & CPR
• NDIS Worker Screening (or Yellow Card)
• Working with Children’s Check (Blue Card)
• Current QLD driver’s licence
• Minimum 6 months of relevant paid experience
