Job Description

We provide person centred community and NDIS related services that help individuals, families and community members access suitable support, participate in community life, and address social, welfare, recreational, health, housing, employment and service access needs.

This is a full time permanent role suitable for someone with strong community services experience who can work with participants, families, community groups, welfare agencies, government bodies and other service providers to identify needs, coordinate support and improve community participation.
About the role

The Community Worker will assess client and community needs, develop and implement support and community participation strategies, liaise with relevant agencies, refer clients to appropriate services, monitor progress, and help clients overcome barriers to accessing welfare, health, housing, employment, education, recreation and community services.

The role is focused on community support, service coordination, advocacy, referrals, program planning and community participation. It is not a direct personal care or disability support worker role.

Main duties
• Assess the needs of clients, families and community members in relation to welfare, health, housing, employment, education, recreation and community participation.
• Interview clients and identify the nature and extent of their social, practical and community access difficulties.
• Develop, implement and review individual support and community participation plans.
• Liaise with welfare agencies, community groups, government bodies, health providers, housing services, employment services and other organisations to coordinate appropriate support.
• Refer clients to suitable agencies and services where additional or specialist assistance is required.
• Promote awareness of available community resources, welfare services and support programs.
• Support clients to access community activities, social programs, recreational opportunities, education, training and employment related services.
• Assist in planning, developing and evaluating community programs and support initiatives.
• Monitor client progress and prepare written reports, case notes and updates in line with organisational and NDIS requirements.
• Advocate for clients where required to improve access to services, reduce isolation and support community inclusion.
• Work with families, carers, service providers and other stakeholders to achieve practical and sustainable outcomes for clients.
• Contribute to continuous improvement of community support programs, policies and service delivery practices.
What we are looking for
• A person with strong knowledge of community services, welfare support and client needs assessment.
• Ability to communicate effectively with clients, families, community groups, government bodies and service providers.
• Experience in case coordination, referrals, advocacy, community participation or welfare support.
• Ability to prepare accurate case notes, reports and service records.
• Good understanding of confidentiality, dignity, cultural sensitivity and professional boundaries.
• Ability to work independently and manage competing client and community needs.
• A genuine commitment to helping people access services, participate in the community and improve their wellbeing.
Qualifications and experience
Preferred qualifications include:
• Diploma of Community Services
• Diploma of Community Development
• Bachelor of Social Work, Community Services, Human Services or similar
• Another relevant qualification in community services, welfare, social work, human services or community development
Applicants with strong relevant experience in community services, welfare support, case coordination, referrals, advocacy or community development will also be considered.
You must have or be willing to obtain
• NDIS Worker Screening Check
• National Police Check
• Working with Children Check, if required
• First Aid and CPR, if required for the role
• Valid driver licence
• Reliable registered vehicle suitable for work related travel
• Right to work in Australia
Pay and conditions
This is a full time permanent position. The successful applicant will be paid in accordance with the applicable industrial instrument. Weekend, public holiday and other applicable penalty rates will be paid where required.
