Description of Duties and Responsibilities
The Medical Case Manager, under the direction of the Program Manager, is responsible for ongoing medical needs assessment, Care Plans, implementation, support, information, referral, and advocacy for individuals in the program. The LPN works collaboratively with residents, case managers, housing support staff, and external healthcare providers to promote health and well-being, manage chronic conditions, and recovery-oriented care.
The Licensed Practical Nurse (LPN) plays a vital role in assisting residents with daily living activities such as eating, bathing, and personal hygiene while also administering prescribed medications, injections, enemas, and performing essential medical tasks like wound care. They support residents’ physical health by monitoring and recording vital signs, including blood pressure, temperature, and heart rate, ensuring that each participant’s SMART Plan includes optimal medical elements. The LPN fosters strong relationships with residents, encouraging self-reliance while providing assistance in managing mental health, substance use, and other health concerns. They play a key role in recognizing and reinforcing positive behavior changes, identifying obstacles that may hinder progress, and working proactively to address them. Additionally, the LPN maintains clear and accurate files, ensuring all resident records are up to date. Collaborating closely with external community supports and program leaders, they facilitate comprehensive care by keeping resident information current and aligned with their evolving needs.
Furthermore, the LPN plays a crucial role in eviction prevention by helping residents manage health-related barriers that could put their housing stability at risk. They work alongside the housing support team to address medical and behavioral concerns that may lead to lease violations, providing education and interventions that promote tenancy success. By ensuring residents have access to proper healthcare, medication management, and harm reduction supports, the LPN helps prevent crises that could result in eviction, fostering long-term housing stability.
Position Qualifications
•Strong understanding of Recovery, Harm Reduction, Client-Centred Care, and Trauma -Informed Care best practices and the willingness and ability to work within this framework.
•Valid registration with the CLPNA
•Valid LPN Practice Permit
•2 to 3 years of direct experience working in a supervisory capacity in a residential setting.
•An open, accepting, and non-judgmental attitude with an ability to build rapport with residents and staff
•Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
•Strong understanding of recovery, client-centred care, and trauma-informed care best practices
•Demonstrated understanding of the addictions field
•Knowledge of referrals resources and health services available for vulnerable populations
•Understanding of inner-city issues
•Familiar with recommendations from the Truth and Reconciliation Commission (TRC) as well as Missing and Murdered Indigenous Women (MMIW) Inquiry
•Current Vulnerable Sector Police Information Check
•Current First Aid/CPR/AED Certification
•Valid Alberta Driver’s License
•Personal vehicle with $2 million liability and ability to transport residents as needed
Hours of work: 3 Month Full-time Contract with a possible extension, Day and Night Shift work. 7.5 Hours Daily, Monday, Tuesday, Thursday, Friday 07:45 4:15 pm, Wednesday 1:45 pm 10:15 pm, No Weekends.
