Salary Range: $95,000 - $115,000 per annum
Position Summary
The Practice Nurse plays a key role within the clinic, delivering a high standard of quality nursing care through efficient, effective, safe, and timely services to YYMS patients. The nurse exercises a significant level of initiative in coordinating, implementing, and monitoring patient care, ensuring appropriate clinical outcomes.
With a strong primary health care focus, the Practice Nurse works collaboratively with other members of the multidisciplinary team to implement preventative health strategies, provide therapeutic interventions, and deliver health promotion and education sessions. The position supports clinical operations and contributes to the overall functioning of the clinical service.
Key Responsibilities
Clinical Care & Assessment
• Triage patients and provide emergency management where required.
• Conduct holistic health assessments, including patient history, health screening, physical examinations, and preventative health checks.
• Support and promote the integration of STI testing into routine primary health care, including adult health checks.
• Administer immunisations in accordance with current guidelines.
• Perform clinical duties within the nurse’s level of competency and scope of practice, following best available evidence.
• Apply nursing assessment skills and practice safely and professionally, aligned with standards and guidelines.
• Work with Medical Practitioners to plan, coordinate, and manage patient care.
• Asist Medical Practitioners during clinical procedures.
• Provide therapeutic care and treatment, including:
o COVID support and management
o Wound care
o Minor surgical procedure assistance
o Medication and Vaccine administration
Diagnostic & Clinical Support Services
• Conduct diagnostic tests such as ECGs, stress tests, hearing checks, spirometry, drug screening, mental health assessments (mood and memory), and other approved procedures.
• Refer clients to appropriate support groups and community services when needed.
• Deliver services that meet Integrated Team Care Program Guidelines.
Patient Experience, Cultural Safety & Community Engagement
• Deliver welcoming, respectful, and professional services for all clients.
• Maintain fairness, confidentiality, equity, and a safe environment for diverse populations.
• Provide culturally appropriate care, incorporating feedback from Aboriginal Health Workers and other Aboriginal staff.
• Practice using a holistic view of Indigenous health.
• Build culturally sensitive health plans that support client ownership and self-determination.
• Provide outreach services including home visits, school visits, and community visits when required.
Leadership, Collaboration and Education
• Provide leadership in nursing and primary healthcare principles.
• Collaborate effectively with the practice team and communicate with external providers and referrers.
• Provide health assessments, health promotion, quality improvement, and population health education.
• Provide training, mentorship, and support to Aboriginal Health Workers.
• Organise case conferences and multidisciplinary meetings as required.
Clinical Administration and Documentation
• Maintain accurate, timely, and clinically appropriate patient documentation in accordance with policy and procedures.
• Collect and enter clinical data into the MMEx system to support complete medical records and accurate reporting.
• Optimise Medicare claiming in accordance with program requirements.
• Manage clinical supplies and resources in line with organisational policy.
• Oversee infection control procedures, patient flow, and clinical processes.
Care Coordination and Client Support
• Work with clients and team members to identify and address broader needs (e.g., accommodation, financial support, wellbeing).
• Facilitate access to essential allied health or specialist services, transport, and GP-approved medical aids.
• Appropriately utilise supplementary services funding for eligible clients.
Selection Criteria
Essential:
• Demonstrated ability as a Registered Nurse in Aboriginal Primary Health Care, including clinical and emergency practice in a remote setting.
• Demonstrated knowledge and skills in primary health care, COVID-19 management, vaccinations, and health education/promotion/prevention programs.
• Demonstrated ability to positively interact with Aboriginal and Torres Strait Islander people to achieve community-desired outcomes.
• Demonstrated understanding of rural and remote health issues, including personal strategies for managing stress.
• Demonstrated understanding of cultural safety and its application in clinical and community settings.
• Demonstrated ability to work effectively as part of a multidisciplinary team, partnering with internal and external service providers.
• Demonstrated knowledge of Standards of Clinical Practice.
• National Police Clearance (or willingness to obtain).
Desirable:
• Current C Class Driver’s Licence.
• Working knowledge and understanding of relevant legislative Acts that impact the organisation and associated accountabilities.
• Local Aboriginal or Torres Strait Islander person.
• Fully COVID-19 vaccinated (evidence required)
