Assessment Nurse (Non-Skilled Agency)

Office location: Huber Heights, OH
Service locations: Montgomery, Miami, Greene and Clark Counties in Ohio

The duties of this position are:
*Open new cases at the client's homes
*Reassess the clients every 60 days with 2 visits being in person and 3 phone visits.
*While assessing the client, you will ask and document personal and health information, collect emergency contact and doctor phone numbers, create a care plan due to the client's needs, check the home for any safety concerns, documenting and turning in paperwork to the office in a timely manner.

This is a part-time position, creating your own schedule within the assessment due date time frame
