FULL TIME REGISTERED NURSE
Salary Range $75,000 to $80,000
Mandatory Qualifications:
• Registered Nurse with current AHPRA registration with the Nursing and Midwifery Board of Australia
Essential KSC:
• Experience in a clinical leadership role in residential aged care
• Good communication, planning, organisational, leadership, analytical and decision-making skills
• Current First Aid and CPR certificates.
• Experience in aged care, preferably in residential and / or community setting.
• A strong commitment to excellence and quality in the provision of aged care related services with a sound working knowledge of quality principles and accreditation.
• Sound practical working knowledge of the ACFI and its application.
• Highly developed leadership skills and the ability to act as a positive role model for staff.
• Strong planning, time management and organisational skills with the ability to prioritise accordingly.
• Excellent people management and interpersonal skills to ensure the achievement of quality outcomes.
• Well-developed communication, problem solving and conflict resolution skills.
• Well-developed interpersonal skills and ability to work with consumers and staff of differing cultural, ethnic and socio-economic backgrounds whilst maintaining role and professional boundaries.
• Able to write and speak English in a way that supports clear communication with consumers and colleagues and government investigative bodies.
Desirable KSC:
• Post graduate qualifications in Leadership, Management and or Aged Care
• Knowledge of the Aged Care Funding Instrument (ACFI)
• Knowledge of the Aged Care Quality Standards
• Tertiary qualifications in gerontology or a related discipline
• Post graduate qualifications in a related Nursing discipline.
• Middle to senior management experience in a health care setting.
• Computer literacy Intermediate.
• Previous experience in the application of quality assurance and or continuous improvement systems.
• Relevant post graduate studies/professional development e.g. continence management, dementia care, health and safety training.
Main responsibilities:
• In the absence of the Site Manager or Clinical Care Coordinator, assume the role of In-Charge’ for the facility and its operations.
• Contributes to the implementation of an effective clinical governance framework for the safe and quality systems required to maintain and improve the consistency, safety and quality of clinical care and to improve outcomes for consumers
• Leads and supports the clinical and personal care and if required activity staff on each shift to consistently deliver safe, quality consumer-centred care
• Promotes a service culture that supports consumers’ health, safety and well-being and is inclusive of their identity, culture and diversity
• Contributes to the efficient allocation and utilisation of resources for safe and quality care
• Appropriate allocation and delegation of care to clinical and personal care staff within their scope of practice and competence including; appropriate delegation of medication administration. Provides clear information that assists staff to complete allocated / delegated care and; provides toolbox training and coaching for skills development as needed
• Promotes a culture of safety that empowers the clinical and personal care workforce to speak up and raise concerns and to work together to seek solutions for consumer safety concerns
• Engages with and fosters partnerships with consumers and or their representative/s when conducting initial and ongoing assessments and care planning.
• Supported decision making to assist consumers to live the life the way they choose. This includes risk assessment and planning with informed consent to support dignity of risk.
• Involves other members of the health team including the lifestyle program staff and other health professionals as appropriate in consultation with the consumers/ representatives
• Plans, allocates and delegates duties to staff according to their training, experience, competence and scope of practice
• Safe administration of medication, including the appropriate delegation to and supervision of Enrolled Nurses and PCAs administering medication
• Consumer assessment includes identifying, assessing and managing risks to minimise preventable harm for each consumer
• Accurate and timely ACFI documentation is completed on each shift
• Care plans are developed, implemented and evaluated in consultation and partnership with consumers/representatives to assist in assessment and planning to assure the consumer gets the care and services needed for their health and wellbeing.
• Conducts monthly care plan evaluation as delegated with input from the team and in partnership with the consumer and or representative.
• Appropriate and timely follow up of exceptions to consumer care plans in partnership with the consumers and or their representative. This includes timely and appropriate referrals to other health professionals and or other services.
• Promotes an evidence-based process for defining infections and the re-enforcement of the Antimicrobial Stewardship Program for the safe and appropriate prescribing and use of antimicrobials
• Consumer deterioration is recognised and timely and appropriate responsive action is taken when a consumer’s health status rapidly declines
• Monitors the areas of key clinical risk and ensures escalation (both internal and external, as relevant) occurs and that appropriate response actions are taken where safety is compromised
• Oversees day to day clinical operations. This includes checking the clinical roster and ensuring appropriate allocations each shift i.e. delegation of care duties.
• Ensures timely and appropriate action in response to incidents, comments and complaints and that transparent and open communication occurs with consumers when things go wrong.
