Salary: $76,000 $80,000 per annum

Westview Disability Care Pty Ltd is an Australian disability support provider operating under the National Disability Insurance Scheme (NDIS). The organisation delivers person-centred support services to individuals with disability, psychosocial conditions, and complex support needs, with a focus on promoting independence, community participation, and wellbeing.
Westview Disability Care provides supports including daily living assistance, community access, and capacity-building services. All services are delivered in accordance with the NDIS Practice Standards, organisational policies, and applicable legislative and regulatory requirements.

The Opportunity
We are seeking a compassionate and reliable Community Worker to support NDIS participants to live independently and participate actively in their communities. This role involves delivering person-centred support in line with individualised support plans, while working collaboratively with participants, families, and support coordinators.
You will assist with daily activities, community access, skill development, and social engagement, ensuring safety, dignity, and respect at all times.

Key Responsibilities
• Assess and support NDIS clients to plan, develop and implement the care/support plans on case-to-case basis
• Interviewing the clients to assess their needs to prepare effective support plans as per their requirements
• Assist clients with daily living activities, community access, and capacity building
• Monitor client’s wellbeing and document progress, incidents, and outcomes accurately
• Refer clients to appropriate community services and allied health supports when required
• Support access to community resources relating to health, welfare, housing, education, and employment
• Liaise with families, carers, support coordinators, and service providers to ensure coordinated care
• Deliver person-centred support in community, group, and supported environments
• Promote independence, social inclusion, and emotional wellbeing
• Maintain accurate records and comply with NDIS Practice Standards and organisational policies

Essential Criteria
• A relevant qualification in public health, aged care, community services, disability or a related field
• Minimum 2 years’ experience providing direct support in aged care, disability, or community-based settings
• A background in Nursing is highly regarded
• Strong communication and problem-solving skills
• Excellent spoken and written English
• Willingness to relocate to, or work in, a regional location and commit to ongoing service delivery
