We are seeking a Full-time and permanent Community Worker who will work closely with individuals, families, and community groups to assess needs, create tailored support plans, and connect people with the resources and opportunities that will improve their quality of life.
Key Responsibilities
1. Assess clients’ needs and develop individualised support programs.
2. Interview clients to understand their challenges and strengths.
3. Plan, deliver, and evaluate community-based programs, workshops, and activities.
4. Monitor client progress and prepare progress reports.
5. Refer clients to external agencies for specialised assistance when required.
6. Assess community needs and identify available resources in areas such as health, housing,
employment, and training.
7. Build partnerships with community groups, welfare agencies, government bodies, and local
businesses to promote awareness of services.
8. Support families and provide education and care for children, young people, and adults in
community-based programs.
9. Assist young people to address social, emotional, and financial challenges.
10. Prepare reports, funding submissions, and documentation to support service delivery
SKILLS
1. Able to assess client needs and prepare individual support plans
2. Interview clients to understand their challenges and strengths
3. Deliver community programs, activities and day to day support
4. Monitor client progress and prepare simple reports
5. Work on a roster across multiple clients
6. Visit clients in their homes and accompany them to appointments
7. Commute between locations throughout the day
8. Work weekends and public holidays including Christmas and New Year’s Eve
9. Follow support plans, maintain boundaries and communicate clearly
10. Penalties will be paid according to the Award
WHAT WE NEED
1. Current driver licence
2. Working With Children Check
3. Police check or willingness to obtain
4. NDIS Worker Screening Clearance or willingness to obtain
5. Minimum Certificate IV in Community Services (or related) with two years relevant experience in the NDIS sector
6. Ability to provide references from previous employers
7. Availability to work shifts, weekends and public holidays
8. Ability to travel between client homes and the community
9. Comfortable supporting clients one to one, including accompanying them outside the home
