As part of the Children’s Behavioral Health Initiative (CBHI) facilitate the process of Intensive Care Coordination (ICC) services. Complete youth assessments that determine strengths, needs, and eligibility for the service. Further develops and facilitates Care Planning Team meetings as well as risk management/safety plans when appropriate. Responsible for coordinating all team member information and meetings, while providing the team with necessary updates that ensure a unified approach in an effort to achieve the family's prioritized vision and goals. Be comfortable with public speaking, be strengths-based and be able to identify the individual and cultural issues relevant to understanding and supporting the needs of the youth and family.
1 year of experience as a psychotherapist or a related occupation and a Master’s degree in Family Therapy and Stress or a related field, or the equivalent as determined by a credential evaluation agency
