Foundation for Innovative New Diagnostics (FIND) is established in India as an independent non-profit created under Section 8 (Indian) Companies Act, 2013 with its office in New Delhi. FIND India is the key implementing partner of Central Tuberculosis (TB) Division, Ministry of Health & Family Welfare, Government of India, for strengthening and expanding TB laboratory diagnostic capacity within the Revised National TB Control Program (RNTCP) supported by the Global Fund. FIND India is a subsidiary of FIND, an international non-profit organization based in Geneva, Switzerland. The organization is dedicated to activities that result in: 1) new diagnostic tools; 2) expanded access to these tools; and 3) strengthened diagnostic testing capacity for poverty-related diseases in low- and middle-income countries. FIND in India has several projects in the area of TB, Hepatitis C (HCV), Malaria, Fevers and Antimicrobial Resistance (AMR).
Multicenter Clinical Study to Assess the Performance of the Xpert MTB/XDR Assay for INH- and Second-line Resistance Detection
In recent years, tuberculosis (TB) control efforts have been complicated by the rise and spread of multidrug resistant (MDR)-TB i.e. TB resistant to the first-line drugs isoniazid (INH) and rifampicin (RIF), and extensively drug resistant (XDR)-TB i.e. MDR-TB with additional resistance to a fluoroquinolone and any of the injectable compounds [amikacin (AMK), kanamycin (KAN) and/or capreomycin (CAP)]1,2. The rapid diagnosis and appropriate treatment of M/XDR-TB is essential to prevent significant morbidity, mortality and further transmission of disease.
Currently used Xpert MTB/RIF assay (Cepheid, Sunnyvale, CA) is only capable of identifying Mycobacterium tuberculosis (MTB) and detecting RIF resistance. The Xpert assay cannot tell whether a patient with RIF resistance remains INH-susceptible, and thus could still be treated with an INH-containing regime. Nor can it identify which RIF-resistant patients can be treated with a FQ and/or aminoglycoside as opposed to those who have XDR TB. Thus, patients are at risk for being either over treated or undertreated, with potentially serious consequences. Therefore, in this study we will be evaluating upcoming innovative 10 colour real time Xpert MTB/XDR assay for INH and second-line resistance detection, and to recommend its use in diverse clinical settings. The focus of this protocol is the multicentre clinical evaluation of Xpert MTB/XDR against MGIT LC-DST, LPA (first and second line) and all controversial results will be further confirmed by sequencing. The Xpert MTB/XDR assay will use the same testing procedures and testing platform as the Xpert, and largely be restricted to patients who already test TB positive (and RIF resistant) in a primary Xpert test. This project will also help in the evaluation of a new test that will make it very easy to rapidly find out who has drug resistant TB and which drugs would be the best ones used in treatment.
The Lab Technicians (2) will be based at National Institute of Tuberculosis and Respiratory Diseases (NITRD), New Delhi.
Identify patients for study enrolment as per project criteria, collect patient’s consent, complete all the CRF, paper work & documentation part as per study protocol
Perform TB diagnostic tests – GeneXpert, Line probe assay (LPA) and liquid culture (LC) and drug susceptibility tests (DST) on patient specimens following standard operating procedures (SOPs) & Good Laboratory Practices (GLP);
Data collection from various experiments performed in the lab, compilation of all the data, data entry, regular data backup etc.
Support to the Junior Research fellow and project manager at NITRD for all laboratory operations as per project needs, preparing periodic project reports in the prescribed formats etc.
Collaborate effectively with project team, participate in trainings, meetings, online discussions etc.
Undertaking any other job assigned by site project manager and the FIND technical team, as per project needs.
Education qualifications and skill requirements/Desired experience:
Bachelors’ Degree in Science with a specialization in Microbiology/ Biotechnology/ Medical Lab Technology, or higher qualification;
Diploma in Medical Lab Technology.
Work experience in TB Culture and DST lab
Rapid molecular TB diagnostics (GeneXpert and LPA) shall be given preference.
Good oral and written communication skills;

Ability to work independently, with minimum hand-holding and supervision;
Energetic, resilient and passionate;
Professional, mature and confident.
Nature of Appointment:
The selected candidate shall be based in New Delhi and will be a Project Linked consultancy contract until 31st Dec 2019.
